NOTICE: Please read this information carefully before submitting your application.

Getting Ready

  • An application for a decision support arrangement requires the completion of identity verification. This applies to all persons that are part of an arrangement application.
  • This form should only be used to verify your ID if you cannot use a MyGovID account or do not have a PPS number .
  • All applications are completed using the MyDSS portal. If you have an accessibility requirement and require a paper form please contact us on 01 211 9750 or at queries@decisionsupportservice.ie.
  • If you are receiving professional support in completing your application, your application must be submitted on the MyDSS portal.

How to complete this application

The process of verifying your identity requires you to complete this online application form and upload copies of documentation that confirm your name and address. Please note:

  • All personal details in this form must match the details on your supporting documents.
  • Incomplete applications will not be processed. You will be notified if this happens.
  • Each applicant should use an email address unique to them. The same email address cannot be used for multiple people.

Documents you need to upload

There are two types of proof of identity that you need to upload as part of your application:

  • One photo ID (current and in date)
  • Two proofs of address (dated within the last six months)

Your uploaded documents must meet the following criteria:

  • Show the front and back of ID cards
  • Photo ID must be in date and not expired
  • Photo ID and proof of address must be fully legible
  • Proof of address must show all four sides and corners of each page (partial documents will not be accepted)
  • All details matching i.e., same name in each document and match details in this form
  • Proof of address must be dated in the last six months

Photo ID must be one of the following:

  • Current valid passport or passport card (from country of citizenship)
  • National Identity Card (EU/EEA/Swiss Citizens)
  • Current valid Irish, UK, or European Driver’s License or Irish Learner Permit (with photo)
  • Irish Public Services Card
  • Irish Government Travel Document (also called a ‘1951 Convention Travel Document’ - for refugees, those with subsidiary protection status, and those recognised as stateless)
  • Irish Certificate of Naturalisation
  • Garda National Immigration Bureau (GNIB) Card
  • Social services card or medical card (with photo)

Proof of address must be two of the following

  • Utility bill e.g., gas, electricity, television, broadband
  • Statement from credit or financial institution. For example, bank statement or credit card statement
  • The applicant’s Notification of Determination of Tax Credits for the current year
  • Household, health, or motor insurance documents (dated within the last 12 months)
  • An official letter or document from a Government Department that includes the applicant’s full residential address
  • Correspondence from the Direct Provision Centre/Emergency Reception & Orientation Centre confirming the applicant’s residency and the date they commenced residency
  • Executed tenancy or property lease agreement
  • Official letter or document on headed paper dated in the last six months
